Insulin Safety Dashboard: A teaching hospital experience in using dashboards to improve safety through a systems thinking approach

Dr Sowmya Gururaj Setty, with Fiona Adlam, Helen Atkins, and Kelly Milton, working with the inpatient diabetes team at University Hospitals of Leicester, and with thanks to colleagues across clinical services who supported this work.

The monthly Insulin Safety Dashboard was used to identify early safety alerts related to inpatient hyperglycaemia. Using the Iceberg Model, a systems thinking tool, we looked beyond the alerts to explore underlying patterns, processes, structures, and assumptions driving high alert rates. This deeper understanding enabled the identification of key leverage points and the delivery of targeted, low-cost interventions to improve insulin safety, strengthen communication, and support safer inpatient diabetes care.
