Type 2 diabetes is one of the most common chronic conditions seen in primary care. Many patients are diagnosed with diabetes through routine screening in primary care; it may also be identified by specialists during a referral for another condition or through screening at hospital if the patient experiences a health issue. At diagnosis of type 2 diabetes, clinicians should assess the patient, set glycemic targets, and initiate the appropriate treatment plan to reach these goals.
