Upcoming Royal College of Physicians events on integrated care for long-term conditions

The Royal College of Physicians is hosting two events in June 2017 that will focus on integrated care for long-term conditions, with a particular focus on diabetes. The events will bring together healthcare and other professionals from primary and secondary care who are developing or delivering integrated care services, to reflect on the barriers and facilitators for successful integration and share learning and experience gained during the integration journey.

 

The events will use experience from an integration project for diabetes care in Oxfordshire as a starting point to highlight the general principles, key themes and learning that are applicable to integrated care across the range of long-term conditions.

 

Thursday 1 June 2017, 6 – 7pm (online audience only): A live streamed panel discussion focusing on the barriers and facilitators to integration. The panel will discuss questions and comments from the web audience. Find out more at: click here

 

Friday 23 June 2017, 10am – 3.30pm at the RCP in London: We invite healthcare and other professionals to join us for an event that will discuss barriers and facilitators in more detail, and drill down into some of the main areas of concern for integration, including IT, patient engagement, clinical leadership, governance/finance and training.
